This is an example of a completed Menstrual record chart (MRC) for a fictitious lady with heavy and painful periods. As you can see, it is completed in calendar months. On the 6th July, Menie Rager began her period and it was very heavy and painful on the 8th, 9th and 10th of July. She had some midcycle pain on the 21st July and having seen her Doctor who prescribed tranexamic acid she had a further period which began on the 4th August. She took the tranexamic acid (indicated by the 1 in the tablets box) from the 4th to the 8th August. Her period was both lighter and less painful. Again she had some mid cycle pain on the 17th August. Her next period began on the 1st September and she took the tranexamic acid again. Once again, her period was much lighter and less painful.

A MRC can be useful in women with other menstrual cycle symptoms as well as ladies attempting to become pregnant. The remarks line can be used to record the severity of symptoms other than pain but I still suggest using the 0 (= no symptoms) to 5 (severe symptoms) scoring system.

It is very useful to start recording your symptoms before you see your Doctor as this is a great help in establishing a diagnosis and planning appropriate investigations and treatment as well as monitoring the effectiveness of any treatment.
